FAQ: IRS Levies and State of Maryland Liens

Where should an Employee's Payroll Deduction Agreement (Form 2159) with Internal Revenue Service be sent?

Send the IRS Form 2159 to the Comptroller of Maryland, Central Payroll Bureau P.O. Box 2396 Annapolis, Maryland 21404 for processing. The Central Payroll Bureau will complete the Employer information located on the left side of the form. The Employee must complete the right side of the form and sign and date it at the bottom. Processing cannot commence unless the form is properly completed.
